A prescription from your doctor is required with a qualifying diagnosis and mobility assessment. Your health plan would need to review the information and authorize the equipment. Please note scooters or power wheelchairs are not to be use in lieu of a car. They are to enhance mobility within the home environment.[/item]

Mon-Fri 8:30am-5:30pm. Twenty- four hour on-call emergency service.[/item]
[item title="Do you deliver on the weekends?"]
CMS only delivers on weekends for hospital discharges and emergency service only.[/item]
[item title="How can I send you the RX if I don’t have a fax machine?"]
Upon delivery of your equipment, our HCT can pick up your prescription.[/item]
[item title="What is the process in order to get equipment?"]
Your physician sends the order to the Medical Director at your Health Plan for review. If there is any additional documentation that is required, they will contact your physician directly. Your health plan will then send the approval to provide the services to us.[/item]
[item title="I have equipment from another company. How do I transition to your company?"]
Contact your current provider and request that your order along with your medical record be sent to CMS. Once we receive the information or signed release, we will make arrangements with you for the transition.[/item]
[item title="Do you provide braces, splints, Diabetics shoes, orthotics or Prosthetics?"]
Not at this time.[/item]
[item title="Do you have a showroom?"]
No.[/item]
[item title="Do I get to choose colors and models of Scooters/Motorized chairs?"]
CMS will only deliver equipment that is covered under your plan and is available in stock. We do not differentiate in color and model. Physician orders that specify a particular make/model must be pre-approved by your Health Plan in most cases. [/item]
[item title="How can I have your company service my scooter/motorized chair?"]
If it is a patient owned scooter/motorized chair, CMS would need to obtain an authorization from your health plan to access the equipment and provide a quote for repairs. This quote will then be submitted to your plan for approval.[/item]
[item title="I’m traveling outside the State what should I do for my oxygen supply?"]
Locate a provider in the area where you will be staying and make arrangements with their company to deliver oxygen services. Always check with your health plan before making any arrangements.[/item]
[item title="What does my insurance cover?"]
Your coverage is based upon the item, Medicare guidelines and the specifics of your Plan’s contract. [/item]
